Educational Activity Form & Resources

The Online Educational Activity (EA) Forms are designed to provide an overview of each educational session within the MD undergraduate program, this is meant as a high level index of educational activities.

The Curriculum Managment Unit (CMU) is concerned with:
  • WHAT is taught (the content, areas of expertise and the learning outcomes)
  • HOW it is taught (the learning resources, delivery of content and learning opportunities)
  • WHEN it is taught (the timetable and curriculum sequence)

Details on the EA Form include:

  1. Session Abstract
  2. Learning Objectives
  3. Keywords
  4. Theme(s) / System(s)
  5. Formulary

Timelines and Escalation Process

Please submit the completed EA form two (2) weeks prior to the start of your teaching week. You can save your online EA form and send the link to another instructor should you want their review or additions.  This is especially useful in sessions where more than one instructor is teaching (often at different sites).

An escalation process involving the week or block chair(s) has been approved in order to obtain the required curricular information.
